This is a Questionnaire intended to map all entities in the supply chain for cocoa. You are asked by your client to provide this information for compliance with the European Union Deforestation Regulation (EUDR).
You will be asked to 'map' suppliers as far back as you can. You will be asked to provide details such as entity name, contact details, address, entity type and activities
Entities are to be mapped by step in the cocoa process. We ask that you add all entities, including traders and other intermediaries.
The details input required for farms will be more extensive and cover data to define the geolocation, a crucial aspect on EUDR compliance.
The questionnaire is built based on the above supply chain
Intermediaries should be added at any point they get involved and could be:
- Exporter
- Trader / Broker
- Government Agent
